Nasi_kerabu : Hi. You can ask the Registrar or the co-sec for the conversion form if you have it in your CDS account. There will be a fee of 20 + tax (21.60).
You will also need to include together the payment (cheque/bank draft) of the total value. It depends on certain company you can do a bank transfer and attach it with the form to the registrar. Details are in the conversion form.
if the universal account is actually nominees account, I'm not sure if ee can instruct moo moo to do it on behalf, there might be handling fees.
